Common Forms Implementation & SciENcv

New “Common Forms” are being adapted by federal sponsors for senior personnel disclosures. Currently, both NIH and NSF require these new forms, and other federal sponsors will follow.

Each agency has their own policies around disclosures, which must be followed. See our Current & Pending, or Other Support page, and scroll down to “Sponsor Specific Guidance.” There you will always find the most recent policy update from each sponsor, including NIH and NSF.

Each individual submitting a Biosketch or Current, Pending & Other Support Common Form must create the form within SciENcv, certify the form, and download the certified copy. You may assign a delegate to assist in preparing each form, but must certify each yourself. Please review SciENcv Guidance for Faculty for instructions on how to prepare your profile for your delegate.

Use each section below to find instructions for SciENcv and each common form.

Guidance for Each Common Form

All common forms are created and certified within SciENcv. This service is hosted by NCBI, and NIH center. You can log into SciENcv using any common credentialing option. While you may assign a delegate to assist with preparing documents, you must connect accounts and assign delegates from your own account. Delegates cannot connect your profile to other accounts.

For NIH Proposals and Disclosures:

Your SciENcv profile must be linked to your eRA Commons and ORCID accounts. If you cannot attach any of these to your account, you may have more than one SciENcv profiles that need merged into one. Submit a ticket to the NCBI help desk to merge profiles and link accounts.

It is critical to ensure these linkages, so that the downloaded certified common form has the proper eRA Commons and ORCID identifiers. Problems with these identifiers will cause a hard stop on NIH proposal and RPPR submissions.

For NSF Proposals and Disclosures:

Your SciENcv profile must be linked to your research.gov account.  Submit a ticket to the NCBI help desk to merge profiles and link accounts.

Assigning Delegates:

From any page within MyNCBI or SciENcv, on the top right, click your name, then “Account Settings.” Scroll down to Delegates, and click the blue “Add Delegate” button. Request delegates by email. They will receive an email request, and confirm access.  Delegates will be listed as “awaiting confirmation” until they have accepted access. If your delegate is listed as “waiting confirmation” but did not receive an email request with a confirmation link, submit a ticket to the NCBI help desk to re-send the delegation email.

In general, grant access to both SciENcv and to MyBibliography, to allow delegates to connect your publications to your biosketch.

For NIH Biosketch Common Form and Supplement:

The NIH requires both the Biosketch Common Form and the Biosketch Supplement.  These are created, certified, and downloaded as a single document from SciENcv. There are several critical changes from the prior Biosketch format, which must be followed.

  • A. Personal Statement – has been moved to the supplement. This will now be limited to 3,500 words. You can discuss five citations only, and the five must be the same five listed under “Products Closely Related to the Proposed Project”
  • B. Positions, Scientific Appointments and Honors – these lists have been moved to the “Appointments and Positions” section on the Biosketch Common Form and the “Honors” section of the Supplement Form. Under appointments, list your current primary appointment history. Include all outside appointments over the prior three years, and optionally include older outside appointments as relevant. Honors will now be limited to 15 entries.
  • C. Contribution to Science – Up to 5 contributions to science can be added, and these can refer to the 5 publications cited above under “Other Significant Products Highlighting Contributions to Science” This is a critical change and strict limit in citations that can be mentioned.

The Supplement Form sections accepting text do not automatically include any formatting. The SciENcv system will accept HTML formatting codes. See guidance for formatting text entries for instructions on how to add HTML formatting, or how to use a  Word-to-HTML converter, can be found

NSF Biosketch Common Form

The NSF Biosketch Common Form does not include a supplement section, and has fewer changes than the NIH version. Any biosketch created from an older NSF version can be updated to the Common Form version. See additional information on the NSF Page for Senior/Key Personnel Documents.

The Current and Pending Support Common Form may be required at the time of proposal, Just-In-Time, or RPPR. See our Policy page on Current and Pending Support to understand what activities need disclosed.

NIH Current and Pending (Other) Support Common Form

Disclosures to NIH are generally required at Just-In-Time or an annual RPPR. NIH requires disclosures of all funded support, along with in-kind support that is valued over $10,000 or is from a foreign source (of any amount). See full NIH Guidance on their Current and Pending (Other) Support page.

NSF Current and Pending (Other) Support Common Form

Disclosures to NSF prior to proposal are required for all funded support, along with in-kind support that is valued over $5,000 or includes committed effort. See additional instructions on the NSF Page for Senior/Key Personnel Documents.
